Foster Application

I am willing to accept the fact that some animals in my care may decline in health and/or behavior, and may require humane euthanasia in instances of terminal illness, and/or behavior affecting their quality of life. I understand that this decision will be carefully evaluated and made by the EHS veterinarian and management, considering the animal's best interest and quality of life.

I understand that anyone interested in adopting my foster animal(s), including me, must go through the standard adoption application process, and the approval of candidates for the placement of the animal(s) is solely determined by the Erie Humane Society.

I have answered all questions in this application truthfully and completely. I understand that although the Erie Humane Society carefully screens animals for foster care placement, it makes no guarantee relating to the health, behavior, or actions of the animals. I understand that I receive foster care animals at my own risk, and can reject or return animals for which the Erie Humane Society has asked me to care for. I indemnify and hold harmless, the Erie Humane Society from all liability arising out of any and all claims, demands, losses, damages, action, judgement, of any and all description which may occur to or be suffered by me, members of my household, or any third parties by reason of activities arising out of this agreement.
